Confidentiality Statement I am an employee of Othram, Inc (Othram). My job duties at Othram require me to perform genetic analysis and / or Forensic Genetic Genealogy (FGG) searches. I am aware Othram has a contract to provide in-house FGG testing for Idaho State Police Forensic Services (ISPFS) and other law enforcement agencies in Idaho under the Idaho FGG contract (the Contract). I am aware ISPFS has submitted a case to Othram under the Contract through Othram's online request system and that case has been assigned request identification number 21728 (the Assigned Case). I agree to the following terms with respect to the Assigned Case and only with respect to the Assigned Case. I have been made aware the Contract requires me to maintain confidentiality of the Assigned Case and not release any information, data, or reports regarding the Assigned Case to anyone other than ISPFS or the investigating law enforcement agency without the express, written pre-approval of the investigating law enforcement agency or a court order. Additionally, I understand that I may not disclose or sell any information regarding my work on the Assigned Case or the results of the Assigned Case outside of Othram and that I will not discuss such work or results with anyone at Othram other than my direct supervisor or others directly working on the case with a signed Confidentiality Statement on file that is substantially similar to this Confidentiality Statement. I agree to abide by the terms of this Confidentiality Statement and recognize any violation will be considered a breach by Othram of the Contract between ISPFS and Othram, Inc. for which Othram will be responsible.
